Part-time Employee Health Insurance Plan

In accordance with the Affordable Care Act (ACA), part-time employees who are regularly scheduled for at least 30 hours per week, or who have averaged more than 30 hours per week over the applicable annual measurement period, are eligible to enroll in the personal choice high deductible plan (medical insurance coverage) and CVS Caremark prescription plan.

Medical Insurance Coverage

This is a personal choice high deductible plan based on a preferred provider organization (PPO) medical plan type. It allows you the freedom of choice to choose doctors and hospitals around the world through the BlueCard Worldwide program. Employees may choose in-network or out-of-network providers. Some services have copays, while others are subject to a yearly deductible. Below is a brief overview of the plan.

Plan Overview

Deductible and Office Visit
  • Fiscal year deductibles of $2,000 for individual coverage; $6,000 for employee plus one (1) or family coverage, which will not be waived at Temple practices.

  • $30 office visit copay to in-network primary care physicians and $40 copay for in-network specialists. Office visit copays are not subject to the deductible, this means that you will pay the above copay regardless of whether you have satisfied the deductible. The office visit copay will be waived at Temple practices.

  • After deductible is satisfied, the plan pays 90% for in-network services such as inpatient hospitalization, surgery, outpatient X-ray/radiology and maternity.

Prescription Coverage
  • Employees pay 10% for the cost of generic drugs, 20% for the cost of preferred brand name drugs and 30% for the cost of nonpreferred brand name drugs. There is a discount for drugs are obtained through mail order. Employees may receive a three (3) month supply for the cost of two months.
  • Prescription drug copays are not subject to the deductible. This means that you will continue to pay the copay amount for prescriptions whether you have satisfied the deductible or not.

Plan Rate(s)

For part time employees eligible for ACA health insurance coverage, below are the monthly employee contribution rate(s) for the personal choice high deductible/CVS Caremark prescription plan. The rate covers both the medical and prescription coverage. Employees electing for health insurance who are not paid every month will need to contact the Benefits Office to make arrangements for payment.
