In accordance with the Affordable Care Act (ACA), part-time employees who are regularly scheduled for at least 30 hours per week, or who have averaged more than 30 hours per week over the applicable annual measurement period, are eligible to enroll in the personal choice high deductible plan (medical insurance coverage) and CVS Caremark prescription plan.
Medical Insurance Coverage
This is a personal choice high deductible plan based on a preferred provider organization (PPO) medical plan type. It allows you the freedom of choice to choose doctors and hospitals around the world through the BlueCard Worldwide program. Employees may choose in-network or out-of-network providers. For part time employees eligible for ACA health insurance coverage, below are the monthly employee contribution rate(s) for the personal choice high deductible/CVS Caremark prescription plan. The rate covers both the medical and prescription coverage. Employees electing for health insurance who are not paid every month will need to contact the Benefits Office to make arrangements for payment.
